The Health Department is located in the lower level of the Municipal Building. The Health Department office is open Monday through Friday from 8:30 AM to 4:30 PM.
Middletown Health Department Mission Statement
The Middletown Department of Health (MDH) is responsible for protecting the health and safety of the people residing or working within our community. The department actively works to prevent disease and promote wellness through planning, education, and a myriad of public health initiatives including (but not limited to): sponsoring health fairs and medical health screenings; conducting regular inspections to ensure compliance with public health codes pertaining to food, air, and water quality, and housing as well as monitoring and responding to public health emergencies, infectious diseases, and environmental health hazards. MDH is also the repository for all City of Middletown birth, marriage, and death certificates.

This is an application only - you will not actually receive your marriage license until both parties appear in our office sometime before the marriage date.
When you come to pick up your license, bring a current photo ID and cash or check for your payment. License: $50.00; Certified Copy: $20.00
Applications for marriage licenses are processed Monday - Friday, 8:30am to 4:00pm (please arrive no later than 4:00pm)

Free Blood Pressure Screenings
Blood pressure screenings conducted by a registered nurse are being offered free of charge by the city health department every Tuesday from 11:30 AM - 12:30 PM in the main lobby. Senior Center Blood Pressure Screenings
Every Thursday; 12:00 PM - 1:00 PM, Middletown Senior Center, 61 Durant Terrace, No appointment necessary, No age or residency requirement, For more information call (860) 638-4540 or (860) 638-4970. Sponsored by the Middletown Health Department.
